COPD

COPD is the second largest cause of emergency hospital admissions in the UK.

Each year, approximately 1 million bed days are accounted for by COPD patients.

Using your practice clinical system and specially designed searches Interface can:

• Reduce symptoms and exacerbations in high-risk patients through pharmacological and non-pharmacological recommendations
• Rationalise the therapeutic management of stable COPD patents
• Reduce practice burden of long-term condition management through provision of Clinical Pharmacist-led patient clinics

The pharmacist will identify COPD patients with persistent symptoms but not showing significant exacerbations. These patients can then be reviewed and action plans produced to manage their treatment going forward.

COPD patients with minimal symptoms & significant exacerbations and COPD patients with persistent symptoms & significant exacerbations will also be reviewed against national guidelines.

COPD patients with a hospital admission managed outside of local guidelines will be identified and flagged to the practice for further review.

Detailed dashboards are presented at baseline and on completion to support the Quality Improvement initiative and PDSA cycles.
